In some cases it was so problematic a reinstall resolved every computer's issue. Going back to your first question, it is indeed a drivers issue. I read an article quite a while back that said Windows 10 was (obviously) trying to be more secure and removing the button from people who are not yet authenticated could possibly exploit that feature, so it was adjusted slightly in that you can access it after you put in your credentials. Also, is the Ease of Access button missing from the lock screen in 10? It's there after Ctrl-Alt-Del, but I thought it was supposed to be there before it as well.
